What We Do

We meet patients where they are. Our RNs and providers visit patients at home and meet them right where they live. We use the latest technology and internet access to provide remote telehealth visits, while the RNs assist patients to make sure there are no issues in getting the care they need.

Any information Life Care’s nurses or remote patient monitoring equipment collects will be shared with your doctor almost immediately. Life Care’s services only work if everyone involved in your care plan is on the same page.
